Purpose: To describe the clinical features with IOFB and analyze prognostic factors affecting the final vision.
Methods:Consecutive medical records of all patients with IOFB admitted to Qingdao Eye Hospital Shandong First Medical University between January 1, 2014, and December 31, 2021, were reviewed retrospectively. Detailed information with a retained Intraocular foreign body was collected according to a standardized data sheet,including age ,sex,trauma causes,the site and length of the wound. Wound entry sites were recorded as zone I (wound limited to the corneal area, including the corneoscleral limbus),zone II (5 mm posterior to the corneoscleral limbus) and zone III (posterior to the anterior 5 mm of the sclera).The characteristics of IOFB injury were recorded in this data sheet.,including the number of IOFBs ,material,longest diameter ,location of IOFBs,and the timing of IOFB removal were recorded .IOFB included the foreign bodies in the anterior segment (anterior chamber, iris, and lens) and the posterior segment (vitreous body, retina, and subretina).And the presence of uveal tissue prolapse, hyphema ,traumatic lens rupture, vitreous haemorrhage , retinal damage,retinal detachment, endophthalmitis,Siderosis bulbi were recorded. All data were collected in an electronic database and crosschecked for errors. Statistical analysis was performed using SPSS V.16.0.
Results:There were 159 patients(160 eyes)with IOFB In this study,and the age of patients were from 10 to 67 years(mean,41.58±11.97years).And males were 151(94.97%),female were 8(5.03%).
All IOFBS occurred at work in our survey.The foreign bodies can show different characteristics,and they cause lacerations in different places,zone1 were 111 eyes(69.81%) ,zoneII 40 eyes(25.16%),zoneIII 8 eyes(5.03%).IOFB located wall of eyeball 13(8.18%),anterior chamber 7(4.40%)crystalline lens 13 (8.18%),vitreous cavity 95(59.75%),in anterior and posterior segments both 4 (2.52%),embedded in retina 21 ( 13.21%),subretinally 5 (3.14%)in orbital 1 ( 0.63%).And the foreign body material was mainly metal,accounted for 90.57%. The foreign bodies varied greatly in size, with the diameter of the largest foreign body being 28 mm. The duration after the injury and before surgical intervention ranged from 1 hour to 20 years and 55% of patients had surgery after24 hours of injury.Details of the IOFB characteristics in the current study can be found.And endophthalmitis was related to traumatic cataract. Final VA was related to site of laceration,length of scleral wound,maximum size of IOFB,Hyphema,vitreous hemorrhage,macular lesions and preoperative visual acutity.
Conclusion:Thus ocular trauma with intraocular foreign body is a common form of ocular injury. The injury is typical among the working age group. The associated risk factor for the visual prognosis were very important.Further a prospective study which provide a efficient treatment to reduce the incidence of posterior segment IOFB.
